The answer is C: An issue is something that is happening and risk is something that might happen. We must figure out how to resolve the issue and move forward. In case of risk, we need to establish mitigation plans. If a new team member joins he should refer to the "Issue Log". The issue log is a project document where all the issues are recorded and tracked. The issues may happen at any time during the project, the issue log is created for the first time as an output of the “Manage and Direct Project Work” process.
